Job summary

Carle Health is seeking an experienced Home Services Social Worker to identify psychosocial needs of patients and families through assessment, and to coordinate resources with interdisciplinary teams. This role requires Hospice experience of 1+ years and offers a sign-on bonus.

Master's Degree in Social Work is required, with LSW or LCSW licensure preferred. You will support patients and families, develop care plans, and document care in the EMR as part of a collaborative team.

Qualifications

• Master's Degree in Social Work required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred.
• Hospice experience of 1+ year is required.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plans of care for family members and caregivers.
• Educate patients and families, addressing barriers and learning styles.
• Document assessments and interventions in the electronic medical record in a timely manner.
• Collaborate with IDT to implement social work plans and support patient decisions.
