Responsibilities

• Perform psychosocial assessments of patients and families to identify emotional, social, and environmental needs.

• Educate patients and families to overcome barriers and align with learning capabilities.
• Document assessments, financial evaluations, and social work interventions in the EMR.
• Collaborate with IDT to develop and implement the social work plan of care.
• Support end-of-life decisions and respect cultural and religious beliefs.
