Job summary

Carle Health is seeking a Home Services Social Worker to identify psychosocial needs of patients and families, conduct assessments, and collaborate with interdisciplinary teams. This role includes hospice-related responsibilities and a sign-on bonus.

Hours may vary with census and program needs. Qualified candidates will have a Master’s in Social Work and licensure, with hospice experience of 1+ years and ability to provide support across the care continuum.

• Ability to work as part of a team.
• Strong assessment and plan-of-care development skills.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care to address family/caregiver needs.
• Document psychosocial assessments and interventions in the patient’s chart.
• Collaborate with IDT to plan and implement social work interventions.
