Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families in home-based hospice care. You will coordinate with interdisciplinary teams to identify resources, provide short-term counseling, and support care planning.

The role requires Hospice experience of 1+ year, a Master’s Degree in Social Work, and licensure (LSW or LCSW). A sign-on bonus is offered.

Qualifications

• Master’s Degree in Social Work is required.
• Licensure as LSW or LCSW is required or eligible.
• Driver’s License and auto insurance are required.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plans of care.
• Support and educate patients and families, considering spiritual/cultural beliefs.
• Document assessments and interventions in EMR.
• Collaborate with IDT and assist with care planning and barriers.
