Job summary

Compassus is seeking a Hospice Social Worker to provide psychosocial assessments, counseling, and care coordination for patients and families. The role collaborates with the interdisciplinary team to develop and implement plans of care, explain Medicare benefits, and assist with long-term care placement when needed.

The ideal candidate holds an MSW from a CSWE-accredited program and licensure for Illinois (LBSW/LMSW/LCSW/LICSW) with healthcare experience.

Qualifications

MSW from CSWE accredited program with at least 1 year of healthcare experience.
State licensure as required for Illinois: LBSW, LMSW, LCSW, or LICSW.

Responsibilities

Explains hospice services and Medicare benefits to patients and families; obtains informed consent and election of benefits documents.
Submits documentation to facilities and ensures timely completion according to policy.
Participates as a member of the IDT; contributes to plan of care.
Provides psycho-social support and counseling to patients and families and coordinates with external counseling resources as needed.
Maintains comprehensive care plans reflecting current problems, goals and interventions.
Supports advance planning including living will/POA documents, funeral arrangements, memorial services, and organ donation.
Assists in placement decisions for long-term care and monitors patient adjustment.
