Job summary

Qualifications

• Master of Social Work (MSW) from a CSWE-accredited program with at least 1 year of healthcare experience.
• Candidates with a bachelor's degree in social work or related field and 1 year of healthcare experience may be considered where MSW supervision is available.
• State licensure as required and applicable for the practice location.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families.
• Provide psycho-social support and counseling to patients and families; coordinate with external resources as needed.
• participate as an IDT member and contribute to the plan of care.
• Complete initial and ongoing psycho-social assessments; address financial, living arrangements, caregiver stressors, and community resource needs.
• Participate in care planning meetings at long-term care facilities; support advance care planning including living wills/POA.
