Job summary

Compassus is seeking a Hospice Social Worker to provide psychosocial assessments, counseling, and support within the interdisciplinary team. The role emphasizes compassionate care, ethical conduct, and coordination with healthcare facilities to ensure patient and family coping and planning.

Responsibilities include educating patients on Medicare benefits, coordinating care plans, and facilitating advance directives while upholding Compassus values and a culture of inclusion.

Qualifications

• Master of Social Work (MSW) from a CSWE-accredited program with at least 1 year of healthcare experience.
• Candidates with a bachelor’s degree in social work or related field and 1 year healthcare experience will be considered for locations with MSW supervision.
• State licensure as required for the practicing state.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain informed consent and election of benefits documents.
• Submit documentation to facilities upon patient visits according to policy.
• Participate as a member of the IDT and contribute to the plan of care.
• Provide psycho-social support and counseling to patients and families; refer to external counseling resources as needed.
• Identify needs for intervention by other IDT members and communicate patient/family needs clearly.
• Coordinate psycho-social care with the IDT to ensure continuity and quality of care.
• Maintain updated, comprehensive care plans reflecting current problems, goals, and interventions.
• Respect family environments and beliefs; maintain nondiscrimination.
