Qualifications

• Master’s in Social Work (MSW) from a CSWE-accredited school is required.
• State licensure is required for this position.

Responsibilities

• Assess psychosocial status of patients and families in terminal illness contexts.
• Provide crisis intervention and counseling to patient/family as needed.
• Educate on culture, advanced directives, and plan of care with the team.
• Coordinate with Interdisciplinary Group and physicians regarding psychosocial factors.
