Job summary

Affinity Hospice Management LLC seeks a licensed Master of Social Work (MSW) to implement standards of care for medical social work services in hospice patients and families. You will be part of the interdisciplinary group, providing psychosocial support based on ongoing assessments and the established plan of care.

The role involves conducting psychosocial assessments, crisis intervention, care planning, and coordination with physicians and community resources.

Qualifications

• Master's in Social Work from CSWE-accredited program.
• Minimum of one year health care or hospice experience preferred.
• Must have a valid driver's license and insured vehicle.

Responsibilities

• Assess psychosocial status of patients and families; communicate findings to the interdisciplinary team.
• Conduct social evaluations including family dynamics and caregiver abilities; provide crisis intervention as needed.
• Educate interdisciplinary team on cultural considerations; assist in preparing advanced directives.
• Participate in developing the individualized care plan with patient and family; attend interdisciplinary meetings.
• Assist in understanding social and emotional factors related to health and death; support funeral planning and practical responsibilities.
• Maintain accurate clinical notes and liaise with physicians and community agencies.
