Affinity Health Management is seeking a Medical Social Worker (MSW) for hospice services in Florida. The role involves implementing standards of care, providing psychosocial support to patients and families, and participating in an interdisciplinary team to monitor goals and interventions.

The MSW will educate families on advanced directives, assist in care planning, and coordinate with physicians and community agencies.

Competenze

• Master’s degree in Social Work (MSW) from a CSWE-accredited program.
• Minimum one year health care experience; hospice experience preferred.
• Demonstrates strong verbal and written communication and organizational skills.
• After offer, satisfactory references and criminal background checks required.
• Must be a licensed driver with reliable vehicle and CPR certification.

Mansioni

• Assess psychosocial status of patients and families, communicate findings to the interdis­ciplinary team, and assist with safe plans in residence.
• Conduct social evaluations, address caregiver needs, and provide crisis intervention as needed.
• Educate on cultural considerations, advanced directives, and care planning.
• Attend interdisciplinary group meetings and support development of individualized care plans.
• Provide continuity of care, coordinate with physicians, and support family coping strategies.
• Participate in on-call duties and ensure resources and equipment are arranged as needed.
