Job summary

Compassus is seeking a Hospice Social Worker to provide preventive, educational, evaluative and treatment services addressing psychosocial needs of patients and families. You will collaborate with the interdisciplinary team to support coping, plan of care, and appropriate utilization of community resources.

Role requires MSW with healthcare experience, state licensure, and a compassionate approach to diverse populations.

Qualifications

• MSW from CSWE accredited program and at least 1 year healthcare experience.
• Bachelor’s degree in social work with 1 year healthcare experience may qualify for certain locations with MSW supervision.
• State licensure as required for practice.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain informed consent and election documents.
• Submit documentation to facilities promptly after patient visits per policy.
• Participate as a member of the IDT and contribute to the care plan.
• Provide psycho-social support and counseling to patient and family; coordinate external resources as needed.
• Identify needs for intervention by other IDT members and communicate with the team.
• Maintain comprehensive care plans reflecting problems, goals, and interventions.
• Facilitate advance planning including living wills, POA, funeral arrangements, and body donation.
