Anchor Health is seeking a Medical Social Worker (MSW) to support hospice patients and families, providing psychosocial assessments, care planning, and coordination through the interdisciplinary team. The MSW will address emotional, social, and practical needs tied to terminal illness, collaborating with nurses and social workers to ensure comprehensive care.

Responsibilities include crisis intervention, education on advanced directives, and resource referrals while maintaining patient records

Qualifications

• MSW accredited by CSWE or a BSW with CSWE accreditation.
• Minimum 1 year social work experience in healthcare; hospice preferred.
• CPR certification current; licensed driver with insured vehicle.

Responsabilités

• Assess psychosocial status of patients and families related to terminal illness.
• Conduct social evaluations, plan interventions, and provide crisis support when needed.
• Educate interdisciplinary team on cultural needs and family dynamics.
• Maintain clinical records for referred patients and coordinate with community agencies.
• Assist with advanced directives education and referrals for environmental needs.
• Participate in plan of care development with patient and family.
