Job summary

Care 360 Hospice in Toledo, OH seeks a licensed social worker to join our hospice team providing medical social services and psychosocial assessments. You will collaborate with the IDG to develop the patient plan of care and ensure timely documentation.

The ideal candidate has an LSW, a CSWE-accredited BS in Social Work, and at least 1 year of healthcare experience; MSW is preferred. A valid driver’s license and reliable transportation are required.

Qualifications

• LSW license in the state of practice
• CSWE-accredited Bachelor's in Social Work required
• One year of social work experience in a healthcare setting
• Valid driver's license, vehicle insurance and access to reliable transportation

Responsibilities

• Provide medical social services to patient and/or family according to the Plan of Care.
• Conduct psychosocial assessments within 5 days of admission.
• Participate in IDG meetings and development of the patient plan of care.
• Observe, record, and report changes in emotional and social factors affecting care.
• Maintain and submit written clinical records including initial evaluation and daily notes.
• Coordinate weekly visits with the patient's case managers for staff scheduling.
