Job summary

OhioHealth at Home in partnership with Compassus is seeking a Hospice Social Worker to support patients and families with psychosocial care, counseling, and resource navigation within the hospice continuum.

You will work as part of an Interdisciplinary Team to assess needs, coordinate care plans, promote advance care planning, and ensure compassionate, culturally sensitive care across settings.

Qualifications

• MSW from an CSWE-accredited program with at least 1 year healthcare experience, or BA in social work with 1 year healthcare experience and MSW supervision available.
• Current state licensure as required for the state of practice.
• Ability to read, write, and speak English; strong communication and counseling skills.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain consent and election documents.
• Submit documentation to facilities in a timely manner per policy.
• Collaborate with the IDT to develop and implement the plan of care.
• Provide psychosocial support and counseling to patients and families; coordinate external resources as needed.
• Facilitate advance care planning, living wills, and POA discussions.
• Participate in care planning meetings at facilities and coordinate transitions of care.
