Qualifications

• Bachelor of Social Work OR Master of Social Work required.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Assists patients and team to understand social factors related to health status.
• Contributes to development of a comprehensive Plan of Care for patients daily.
• Instructs on community resources available to assist patients as needed.
