Job summary

Compassus is seeking a Hospice Social Worker to address psychosocial needs of patients and families, collaborating with the IDT to plan care and coordinate resources. The role includes counseling, assessment, and facilitating advance planning, with emphasis on compassionate, non-discriminatory care across settings.

The position requires an MSW from a CSWE-accredited program and Michigan licensure or eligibility, with at least 1 year of healthcare experience.

Qualifications

• MSW from CSWE-accredited program; healthcare experience (1+ year).
• Bachelor's degree in social work or related field with 1 year healthcare experience (supervision option).
• State licensure as required in Michigan: LBSW/LMSW/LCSW/LICSW.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families; obtain informed consent and election documents.
• Submit documentation to facilities after visits; ensure timely completion per policy.
• Participate in the Interdisciplinary Team (IDT) and contribute to care plans.
• Provide psycho-social support and counseling to patient and family; coordinate external resources as needed.
• Manage and update comprehensive care plans reflecting goals and interventions.
• Support advance planning (living will/POA, funeral arrangements, body donation) and placement in long-term care when needed.
