Job summary

Serenity Hospice is seeking a licensed social worker to deliver varied social work services to patients and families, including initial emotional, spiritual and psychosocial assessments, ongoing counseling, bereavement support, outreach and referrals as part of the Hospice IDG.

The role requires Ohio licensure, an MSW from a CSWE-accredited program, and experience in healthcare; candidates should be able to work independently, manage crises, and collaborate with a multidisciplinary team while

Qualifications

• MSW degree from CSWE-accredited program.
• Ohio social work license.
• Minimum 2 years in healthcare social work (preferred).

Responsibilities

• Provide initial psychosocial, emotional, spiritual and bereavement assessments.
• Deliver ongoing counseling and support to patients and families.
• Coordinate referrals and community resources as part of the IDG.
• Document visits and interactions in patient records.
• Participate in Bereavement Team activities and staff meetings.
