Job summary

Ohioans Home Health and Hospice seeks a compassionate Licensed Social Worker in Toledo to join our interdisciplinary team. The role focuses on psychosocial assessments, advocacy, and comprehensive care planning for patients and families in home health and hospice settings.

Requirements include a CSWE-accredited degree and an Ohio Social Worker license (Michigan license optional). Minimum one year in health care, strong communication, and a patient-centered approach are essential.

Qualifications

• Bachelor's or Master's degree accredited by the CSWE.
• Current Licensed Social Worker license in Ohio, willingness to obtain Michigan license.
• Minimum 1 year healthcare experience, preferably in home health or hospice.
• Understanding of hospice philosophy and death/dying issues.
• Ability to relate to a diverse population and build professional relationships.
• Strong verbal and written communication skills.

Responsibilities

• Conduct psychosocial assessments and interventions for patients and families.
• Collaborate with interdisciplinary teams to plan individualized care.
• Advocate for patient needs and coordinate support services.
• Educate about advance directives and end-of-life planning.
• Assist with funeral, financial, legal, and healthcare decision planning.
• Document care plans and participate in routine interdisciplinary meetings.
