Compassus is seeking a dedicated Hospice Social Worker in Wisconsin to provide psychosocial care for patients and families within an interdisciplinary team. You will explain hospice benefits, support families, and coordinate with community resources to promote optimal coping and planning.

The role requires an MSW from a CSWE-accredited program with at least 1 year of healthcare experience and state licensure. Strong communication and counseling skills are essential for success in this position.

Qualificações

• MSW from a CSWE-accredited program with at least 1 year of healthcare experience.
• Bachelor's degree in social work with 1 year healthcare experience will be considered for locations with MSW supervision.
• State licensure as required and applicable for the state of practice.

Responsabilidades

• Explains hospice services and Medicare benefits to patients and families; obtains informed consent and election of benefits documents.
• Submits documentation to facilities promptly after patient visits and per company policy.
• Participates as a member of the IDT and assists in developing and implementing the plan of care.
• Provides psychosocial support and counseling for patients and families; coordinates with external resources as needed.
• Identifies when intervention by other IDT members is required and communicates needs effectively.
• Maintains comprehensive care plans reflecting current problems, goals, and interventions; ensures non-discrimination.
