Job summary

Compassus is seeking a Hospice Social Worker to deliver psychosocial services to patients and families, coordinating with the interdisciplinary team to support coping and plan of care. The role requires an MSW (CSWE-accredited) with healthcare experience or a BS in social work with supervision eligibility, and state licensure where applicable.

Qualifications

• Master of Social Work (MSW) from a CSWE-accredited program with at least 1 year of healthcare experience.
• Alternatively, a BS in social work or related field with 1 year of healthcare experience will be considered where MSW supervision is available.
• Candidates must hold state licensure as required for the practicing state.

Responsibilities

• Explain hospice services and Medicare benefits; obtain informed consent and election documents.
• Submit documentation to facilities promptly after patient visits per policy.
• Participate as IDT member in planning and implementing the care plan.
• Provide psychosocial support and counseling; coordinate external resources as needed.
• Identify needs for intervention by other IDT members and communicate with the team.
• Maintain comprehensive care plans reflecting problems, goals, and interventions.
• Facilitate advance planning including living wills, POA, and end-of-life arrangements.
