Job summary

Compassus is seeking a Hospice Social Worker in Michigan to deliver psychosocial assessments and counseling within a multidisciplinary team. You will explain hospice benefits, coordinate with facilities, and support advance care planning for patients and families.

The role requires an MSW from a CSWE-accredited program, Michigan licensure, and healthcare experience. You will contribute to care planning and ensure compassionate, compliant psychosocial care.

Qualifications

• MSW from CSWE-accredited program with healthcare experience.
• Must hold state licensure as required in Michigan (LBSW, LMSW, LCSW, or LICSW).

Responsibilities

• Explains hospice services and Medicare benefits to patients and families and documents appropriately.
• Participates as a member of the IDT and contributes to the plan of care.
• Provides psycho-social support and counseling and connects to external resources as needed.
• Maintains comprehensive care plans reflecting problems, goals, and interventions.
• Facilitates placement in long-term care and participates in care planning meetings.
• Supports advance care planning including living will/POA and funeral arrangements.
