Job summary

Compassus is seeking a Hospice Social Worker to deliver psychosocial care, collaborate with the interdisciplinary team, and support patients and families in navigating the complexities of end-of-life care. The role emphasizes advance planning, grief support, and coordination with community resources to improve coping and maintain quality of life for patients and families served by Compassus.

Qualified candidates hold an MSW from a CSWE-accredited program with healthcare experience, or a BS

Qualifications

• MSW from CSWE accredited program with at least 1 year healthcare experience.
• Bachelor's degree in social work or related field with 1 year healthcare experience will be considered for locations with MSW supervision.
• State licensure as required for practice in the practice state.

Responsibilities

• Explains hospice services and Medicare benefits to patients and families; obtains informed consent and election of benefits documents.
• Submits documentation to facilities after visits per policy.
• Participates as a member of the IDT and develops/implements the plan of care.
• Provides psycho-social support and counseling to patient and family; coordinates with external counseling resources.
• Identifies need for intervention of other IDT members and communicates patient needs to IDT.
• Coordinates psycho-social care with IDT for continuity and quality of care.
• Maintains comprehensive care plans reflecting problems, goals, and interventions.
• Supports advance planning including living will/POA, funeral arrangements, and body/organ donation.
