Job summary

Bon Secours Home Care and Hospice by Compassus seeks a Hospice Social Worker to support psychosocial care, collaborate with the interdisciplinary team, and assist patients and families in coping and planning for end-of-life care.

The role includes counseling, care planning, and coordination with community resources, with emphasis on ethical conduct and nondiscrimination. Prior MSW with healthcare exposure is preferred; licensure as required by state is needed.

Qualifications

• MSW from a CSWE accredited program with at least 1 year of healthcare experience.
• Bachelor's in social work with 1 year healthcare experience may be considered where MSW supervision is available.
• State licensure as required for the state of practice.

Responsibilities

• Explains hospice services and Medicare benefits to patients and families; obtains informed consent and election of benefits documents.
• Provides psycho-social assessments and counseling to patients and families; coordinates with the IDT for care planning.
• Maintains comprehensive care plans reflecting problems, goals and interventions.
• Participates in care planning meetings at long-term care facilities and supports advance planning including living wills/POA and funeral arrangements.
