Job summary

Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Social Worker to support patients and families, collaborating with the interdisciplinary team to plan and deliver psychosocial care. The role emphasizes compassionate, ethical practice within Compassus’ pillars.

Responsibilities include counseling, care coordination, and facilitating advance planning while upholding state licensure requirements and organizational policies.

Qualifications

• MSW from a CSWE-accredited program with at least 1 year of healthcare experience.
• State licensure as required for the state you practice.
• Bachelor's degree in social work with 1 year healthcare experience may be considered for locations with MSW supervision.

Responsibilities

• Explain hospice services and Medicare benefits; obtain informed consent and election of benefits documents.
• Submit documentation to facilities after visits according to policy.
• Participate as IDT member; develop and implement plan of care.
• Provide psycho-social support and counseling; connect with external resources as needed.
• Identify needs for IDT intervention and communicate patient/family needs.
• Coordinate psycho-social care with IDT to ensure continuity and quality.
