Job summary

Compassus is seeking a Hospice Social Worker to address psychosocial needs of patients and families, integrating services with the interdisciplinary team to optimize coping and outcomes.

You will assess, plan, and implement social work interventions, coordinate with facilities, and support advance planning. A CSWE-accredited MSW with healthcare experience and state licensure is required or supervision available where MSW is present.

Qualifications

• MSW from a CSWE-accredited program with at least 1 year of healthcare experience.
• Bachelor’s degree in social work or related field with 1 year of healthcare experience (supervision available at MSW locations).
• State licensure as required for the practice state.

Responsibilities

• Explain hospice services and Medicare benefits; obtain consent and election documents.
• Submit documentation to facilities post-visit; ensure timely completion per policy.
• Participate in IDT; develop and implement the care plan.
• Provide psychosocial support and counseling; link to external resources as needed.
• Identify interventions needed from other IDT members and communicate patient needs.
• Coordinate psychosocial care with IDT for continuity and quality of care.
• Maintain up-to-date care plans reflecting problems, goals, and interventions.
• Support advance planning including living will/POA and funeral arrangements.
