Qualifications

• MSW from a CSWE-accredited program with at least 1 year healthcare experience.
• Bachelor’s in social work with 1 year healthcare experience may be considered for supervision.
• Licensure required: LMSW, LCSW, or LICSW.

Responsibilities

• Explain hospice services and Medicare benefits; obtain informed consent.
• Submit documentation to facilities per policy.
• Participate in IDT and contribute to care plan development.
• Provide psycho-social support and counseling to patients and families.
