Qualifications

• Master's Degree in Social Work is required.
• Licensure as LCSW or LSW in Illinois (IDFPR) is required.
• Valid driver's license and auto insurance are required.
• Experience in hospice and palliative care is preferred.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional and social needs.
• Develop a bereavement plan of care addressing family needs.
• Educate patients and families about learning needs and cultural beliefs.
• Complete and transmit visit notes and orders timely per policy.
• Collaborate with IDT to develop and implement social work care plans.
• Support patient decisions and self-determined life care choices.
• Assist with transportation barriers and referrals for benefits and resources.
• Advocate for patient and family dignity and cultural values.
