Qualifications

• Master's degree in Social Work required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) credentialing indicated.
• Driver's license required.
• Auto insurance proof required.
• Hospice experience preferred.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop bereavement plan of care addressing family needs.
• Interpret and communicate patient/family faith and cultural traditions.
• Educate patients and families to overcome barriers and meet learning needs.
• Document psychosocial assessments, financial assessments and MSW interventions in EMR.
• Collaborate with IDT to develop social work plans of care.
• Identify and address transportation, financial, and resource barriers.
• Make referrals for Medicaid, Community Care, SSDI, and elder abuse/neglect as needed.
• Respond to imminent death indicators and support patient/family during end-of-life.
