Qualifications

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop and implement a bereavement plan of care addressing family member needs.
• Coordinate with IDT and collaborate with interdisciplinary teams to support care planning.
• Educate patients and families to overcome barriers and learn about resources and benefits.
• Document assessments and interventions in the EMR and ensure timely updates.
