Qualifications

Responsibilities

• Perform psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop bereavement plans of care addressing family/caregiver needs.
• Educate patients and families respecting faith and cultural beliefs; support decision making and end-of-life care planning.
• Document assessments and interventions in the EMR; coordinate with the IDT for care planning.
• Assist with transportation, referrals, and access to financial aids and community resources.
• Ensure care aligns with Medicare Hospice benefit requirements and authorization processes.
