Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs of patients and families and coordinate with interdisciplinary teams. The role includes developing bereavement plans and addressing barriers to care while documenting in the EMR.

Qualifications include MSW, LCSW/LSW licensure, and a valid driver's license with auto insurance. Hospice experience is preferred and hours vary with census and program needs.

Qualifications

• Master’s degree in Social Work (Required).
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) license.
• Driver’s License and auto insurance.
• Experience in hospice is preferred.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems.
• Develop a bereavement plan of care addressing family/caregiver needs.
• Educate patients and families about cultural, faith, and religious considerations and how they affect care.
• Document psychosocial assessments and interventions in the patient’s electronic medical record.
• Coordinate with the interdisciplinary team to implement and adjust the social work plan of care.
