Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care addressing family needs.
• Educate patients and families, respecting beliefs and cultural traditions.
• Document assessments and interventions in the electronic medical record.
• Collaborate with IDT to implement the social work plan of care.
