Qualifications

• Master's degree in Social Work is required.
• Licensed Social Worker (LSW) and/or Licensed Clinical Social Worker (LCSW) licensed in IL (IDFPR).
• Hospice experience of 1+ years is required.

Responsibilities

• Provide psychosocial assessments of patients and families.
• Develop a bereavement plan of care for families and caregivers.
• Address spiritual and cultural considerations in care planning.
• Educate patients and families and overcome barriers to learning.
• Document assessments and social work interventions in eMR in a timely manner.
