Job summary

Qualifications

• Master's degree in Social Work required.
• Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) licensure.
• Hospice experience of 1+ year.
• Driver's license and auto insurance proof may be required.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care addressing family needs.
• Educate patients and families and overcome barriers; tailor teaching to learning capabilities.
• Complete visit notes and orders timely according to policy.
• Document psychosocial assessments and interventions in the electronic medical record.
• Implement a social work plan that strengthens family systems and maximizes patient coping.
