Job summary

Carle Health in Florida is seeking a Home Services Social Worker to assess psychosocial needs of patients and families, coordinating care with interdisciplinary teams.

The role requires hospice experience (1+ year), with focus on resource identification, bereavement planning, and culturally sensitive support, plus a sign-on bonus.

You will document assessments in the EMR, collaborate with IDT, and ensure care plans respect patient and family beliefs while maximizing benefits.

Qualifications

• Master's Degree in Social Work required.
• Field of Study: Social Work.
• Licensure requirements include LSW and LCSW per applicable state regulations.
• Driver's license required.
• Ability to work as part of a team.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and life situation.
• Develop a bereavement plan of care to address family member/caregiver needs.
• Interpret and communicate pt/family faith and culture traditions and consider how beliefs impact care.
• Educate patients and families in overcoming barriers and tailoring education to learning styles.
• Document psychosocial assessments, financial assessments, and interventions in the EMR.
• Implement social work plans to enhance family support, resource utilization, and benefits.
• Collaborate with the IDT to develop and adjust care plans.
• Respect patient and family decisions and maintain culturally sensitive care.
