Job summary

Carle Health is seeking a Home Services Social Worker to assess psychosocial needs and collaborate with interdisciplinary teams. The role requires Hospice experience of 1+ year and offers a sign-on bonus.

Responsibilities include psychosocial assessments, bereavement planning, and facilitating education for patients and families while documenting care in the EMR and coordinating with the IDT to optimize benefits and environmental support.

Qualifications

Master's degree in Social Work required.
Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) per state regulations.
Hospice experience of 1+ year is required.
Driver's license and auto insurance required.
Ability to work as part of an interdisciplinary team.

Responsibilities

Provide psychosocial assessments of patients and families to identify needs.
Develop bereavement plans of care addressing family needs.
Communicate patient/family beliefs and cultural considerations to the care team.
Educate patients and families to overcome barriers and meet learning needs.
Document assessments and MSW interventions in the electronic medical record.
Implement social work plans to maximize benefits and coping.
Support patient decisions and collaborate with the IDT on care plans.
Identify indicators of imminent death and address end-of-life needs.
