Job summary

Qualifications

• Master's degree in Social Work required.
• Licensed Social Worker (LSW) and Licensed Clinical Social Worker (LCSW) licensure required.
• Driver's license and proof of auto insurance required.
• Hospice experience of 1+ years required.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs related to diagnosis and care.
• Develop a bereavement plan of care addressing caregiver needs.
• Educate patients and families and address barriers to learning.
• Complete visit notes and orders timely according to policy.
