Zusammenfassung

UnitedHealth Group's Amedisys Hospice team seeks an On-Call RN to provide after-hours clinical support across Mon County, with remote triage and field visits. You will assess patient needs, coordinate with physicians and care teams, educate patients and caregivers, and document all decisions.

Requirements: current RN license in the assigned state, CPR certification, 1+ year hospice/home health or acute care, and a valid driver's license.

Qualifikationen

• Active RN license in the state(s) you are assigned to work.
• 1+ year of nursing experience, preferably in hospice, home health, or acute care
• Current CPR certification and valid driver’s license are required.

Aufgaben

• Provide on-call nursing support, triaging patient needs and determining interventions.
• Assess patient conditions via phone and/or in-person visits and escalate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
