Job summary

UnitedHealth Group is seeking an Afterhours Registered Nurse to deliver comprehensive hospice care to patients and families during nights and weekends, including after hours. The role focuses on triage by phone, in-home visits when needed, and timely documentation to ensure quality care.

Applicants must hold an unrestricted RN license in the state of practice, a valid driver’s license, and access to reliable transportation.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Triage patient needs over the phone until an in-person visit by a nurse is available.
• Respond urgently to crisis calls from patients and families after hours.
• Perform initial nursing evaluation within 48 hours of referral.
• Conduct afterhours visits including admissions, death, recertification, and routine visits.
• Identify and reassess patient/family needs every 15 days or as needed.
• Document problems, goals, interventions, and patient/family response to care.
