Job summary

Heart to Heart Hospice is seeking a Registered Nurse On Call to provide home visits for patient assessment, treatment and support, including crisis management and potential pronouncement of death. You will collaborate with physicians and the IDG to develop and revise comprehensive nursing care plans and document progress.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State Registered Nurse License.
• Current Driver's License with reliable transportation and ongoing auto liability insurance.
• Two years of experience as an RN (preferred in healthcare/clinical setting).

Responsibilities

• Provide home visits as needed for patient assessment, treatment, and support, including pronouncement of patient death and crisis management.
• Plan comprehensive service and revise nursing care plans with physician and IDG.
• Use excellent customer care skills with internal and external customers, including phone etiquette.
• Prepare clinical/progress notes showing progress toward goals.
• Inform physician of changes in patient needs and outcomes; evaluate patient/family response to care.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and internal policies.
