Job summary

Moments Hospice is seeking an on-call registered nurse to join our team for nights and weekends, providing after-hours hospice care in patients' homes with emphasis on compassionate clinical leadership. The role supports the hospice interdisciplinary group and patient families.

This full-time, on-the-road position follows a 7 days on/7 days off schedule, with shifts from 5:00pm through 8:30am on weekdays and 5:00pm to 8:30am on weekends. Competitive benefits accompany the role.

Qualifications

• Licensed as a Registered Nurse (RN) in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Perform initial and ongoing comprehensive patient assessments for hospice needs.
• Respond to calls from Triage during shifts and visit patients with changing conditions.
• Provide professional nursing care per the nursing process.
• Develop and revise care plans in collaboration with the team.
• Coordinate with physician and community health providers.
• Educate patient and family on self-care and medications.
• Maintain accurate clinical documentation.
• Ensure a safe environment and after-hours continuity of care.
• Participate in interdisciplinary group meetings.
• Manage equipment and service arrangements as needed.
