Job summary

Moments Hospice seeks an on-call Registered Nurse to coordinate after-hours hospice care in the Mankato area. You will assess patients, respond to triage calls, and provide physician-guided nursing care in home settings. This full-time role supports families and communities with compassionate, expert care.

The position follows a 7 days on/7 days off schedule with evening and weekend shifts. A company car with fuel/insurance is included, along with health, dental, vision, PTO, and a 401(k) plan.

Qualifications

Licensed RN in Minnesota.
Experience with Electronic Medical Records (EMR).
Strong planning, organizing, and communication skills.
Valid Driver's License and reliable vehicle.

Responsibilities

Conduct initial and ongoing patient/family assessments for hospice needs during after-hours.
Respond to triage calls and arrange visits for changing patient conditions.
Provide nursing care per the plan of care and physician orders.
Develop and revise care plans based on patient status and nursing diagnoses.
Coordinate with physicians and interdisciplinary team to ensure continuity of care.
