Job summary

Moments Hospice seeks a full-time on-the-road RN to provide after-hours hospice care in the community. You’ll complete comprehensive assessments, develop and revise care plans, administer medications, and collaborate with physicians and the interdisciplinary team.

This role includes 7 days on, 7 days off, with evenings and weekend coverage. A company car is provided for business and personal use at minimal cost, along with a robust benefits package.

Qualifications

• Licensed RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously while maintaining high standards.

Responsibilities

• Performs initial, comprehensive assessments and ongoing re-evaluations.
• Responds to after-hours calls and makes visits as patient conditions change.
• Administers medications and treatments per physician plan.
• Counsels patient and family on self-care and care plans.
• Coordinates with physicians and the hospice interdisciplinary team.
