Moments Hospice is seeking a dedicated Registered Nurse to join our on‑call hospice team serving patients in Minnesota. The role emphasizes after‑hours coverage, including nights and weekends, with a 7 days on/7 days off schedule.

Qualifications

• Licensed as a Registered Nurse / RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance.

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice care needs for after-hours calls.
• Responds to calls from Triage during assigned shifts and visits patients with changing conditions.
• Provides professional nursing care using the nursing process.
• Develops and revises the plan of care with patient and family participation.
• Administers medications and treatments as prescribed by the physician.
• Counsels the patient and family in nursing care and health management.
• Maintains a safe environment and ensures continuity of care.
• Attends interdisciplinary group meetings and coordinates with the hospice team.
