Job summary

Moments Hospice is seeking an on-the-road Registered Nurse for after-hours coverage and home-visiting care. The role includes comprehensive nursing assessments, care planning, and coordination with physicians and families in patients’ homes.

You will work 7 days on/7 days off, with evenings and weekend shifts on a rotating schedule. The position offers a company car with fuel and insurance covered, competitive salary, and benefits including health, dental, vision, PTO and 401(k) matching.

Qualifications

• Licensed as a Registered Nurse / RN in the state you work in.
• Proficiency with Electronic Medical Records (EMR) software.
• Excellent planning, organizing, and communication skills.
• Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
• Valid Driver's License and reliable vehicle with current insurance

Responsibilities

• Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs for after-hours calls.
• Available to receive and respond to calls from Triage during assigned shifts making visits to patients with changing conditions, and completing weekend admissions.
• Provides professional nursing care by utilizing all elements of nursing process.
• Assesses and evaluates patient’s status by writing and initiating plan of care, re-evaluating needs, and revising plans as necessary.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Uses health assessment data to determine nursing diagnosis and develops a care plan with patient/family involvement.
• Delivers medications and treatments as prescribed and provides health counseling to patient and family.
• Maintains a safe environment and coordinates with interdisciplinary team for continuity of care.
