Job summary

Moments Hospice seeks an on-call registered nurse (RN) to organize and direct hospice care during nights and weekends, covering calls, visits, and assessments in home settings. This full-time role requires clinical expertise, strong communication, and the ability to work autonomously in fast-changing situations.

You’ll join a team focused on compassionate patient care, care planning, family support, and adherence to EMR documentation.

Qualifications

• Licensed as a Registered Nurse in the state you work in.
• Proficiency with EMR software.
• Excellent planning, organizing, and communication skills.

Responsibilities

• Complete initial and ongoing patient/family assessments to determine hospice needs.
• Respond to after-hours calls from triage, visiting patients with changing conditions.
• Provide professional nursing care according to the nursing process.
• Develop and revise care plans with patient and family involvement.
• Administer medications and treatments as prescribed.
• Coordinate with physician and community for continuity of care.
