Job summary

Amedisys, part of the Optum family, is seeking a Hospice RN On-Call to provide after-hours clinical support across home-based and field settings. You will triage patient needs, assess conditions by phone or visit, coordinate with physicians and care teams, and educate patients and caregivers while documenting care decisions.

The position offers a full-time salary range from $71,640 to $107,460 annually, plus a comprehensive benefits package, incentive programs, equity stock purchase and 401k

Qualifications

• Current RN license, applicable to assigned state(s).
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• 1+ year nursing experience in hospice, home health, or acute care.

Responsibilities

• Provide on-call nursing support, triaging patient needs and interventions.
• Assess patient conditions via phone or in-person visits and escalate as needed.
• Coordinate with physicians, care teams, and caregivers for timely care delivery.
• Educate patients and caregivers on symptom management and care plans.
• Maintain accurate documentation of all patient interactions and clinical decisions.
