Heartland Hospice is seeking an After Hours / On-Call Registered Nurse to respond to patient needs during evenings, nights, weekends, and holidays, delivering compassionate care in patients’ homes. You will collaborate with a multidisciplinary team to implement the plan of care and ensure seamless continuity of care.

The role requires an active RN license, at least 1 year of nursing experience, CPR certification, and reliable transportation for home visits.

Qualifications

• Current RN license in the state of intended employment
• Minimum 1 year of nursing experience
• CPR certification
• Valid driver’s license and reliable transportation

Responsabilités

• Provide direct patient care after normal business hours
• Serve as primary liaison between patients, families, physicians, and hospice team during your shift
• Respond promptly to urgent calls, patient visits, and symptom management needs
• Document visits and communications accurately
• Participate in quality assessment and performance improvement programs
• Work independently while coordinating with the daytime care team
