Heartland Hospice is seeking an After Hours / On-Call Registered Nurse to provide compassionate care in patients' homes during evenings, nights, weekends, and holidays. You will respond to urgent needs, participate in care planning, and coordinate with the daytime team to ensure continuity of care.

The role requires an active RN license in the state, 1+ year nursing experience, CPR certification, and a valid driver’s license.

Qualificações

• Current RN license in the state of employment.
• 1+ years of nursing experience.
• Current CPR certification.

Responsabilidades

• Provide direct patient care after normal business hours.
• Serve as primary liaison between patients, families, physicians and the hospice team.
• Document visits and communications accurately.
• Respond promptly to urgent calls and symptom management needs.
• Collaborate with interdisciplinary team to deliver comprehensive care.
