Job summary

Chapters Health System is seeking an experienced RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the interdisciplinary team, and provide clinical, palliative and supportive care to keep patients in their home environment as long as possible. You will manage admissions, educate families, and coordinate with physicians to ensure seamless care delivery.

A per diem schedule with potential 7-on/7-off shifts is anticipated for initial leave coverage, with mileage

Qualifications

• Current license as RN in the state where the employee will be working.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Minimum one (1) year nursing experience in hospice or hospital experience preferred.
• Previous experience working with an EMR/EHR system.
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy.
• Reliable transportation to meet visit schedule.
• Ability to use equipment with visual and auditory mechanisms.
• Flexible schedule to meet the needs of Hospice House patients.
• Ability to manage the emotional stress of working with and caring for terminally ill patients and their families.
• Ability to physically access patient rooms, navigate stairs and narrow spaces, and meet physical demands.

Responsibilities

• Provides and manages direct care to patients and families as part of IDG, with appropriate nursing intervention and follow-up.
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patient/family on disease, palliation, care giving, and safety practices.
• Reports changes in condition to IDG or other health professionals.
• Presents concise oral and written reports to IDG and coordinates timely referrals.
